Nursing Homes
Let our consultants guide you to create integrated dialysis programs within nursing homes, ensuring comprehensive care for elderly dialysis patients.
The number of geriatric end-stage renal disease (ESRD) patients in the US is rising disproportionately compared to other age groups on dialysis. Consequently, more dialysis patients will need the assistance of nursing homes or extended care facilities. While nursing homes can aid in social and physical rehabilitation, many lack the capacity to properly care for dialysis patients. These patients have higher rates of peritonitis on peritoneal dialysis (PD) and higher mortality rates on both hemodialysis (HD) and PD compared to community dialysis patients. Effective cooperation and communication between dialysis centers and nursing homes are essential due to the complex management required for these patients.
One promising approach is integrating nursing home and dialysis unit services.
Consider these 3 areas to determine if initiating a dialysis for elderly nursing home patients is appropriate for your team:
Don’t limit the discussion to the glomerular filtration rate (GFR), a measure of renal function. Instead, that dialysis may be necessary to improve conditions such as refractory hyperkalemia or fluid overload that is unresponsive to diuretics. Conversely, if the underlying issues such as adult failure to thrive or cognitive impairment will not be improved by dialysis, it may not be appropriate despite the GFR.
Appreciate the life expectancy of the resident, considering both functional and chronological age. For example, the life expectancy of an 85-year-old on dialysis is approximately 12 months.
Determine the resident’s goals. Is the focus on symptom management, avoidance of hospitalization, or the extension of life.
